約會不能只做到表面功夫，床上的表現也要兼顧！為了解台灣男性勃起功能現況及兩性性愛認知，台灣男性學醫學會進行《硬不硬有關係！男性勃起硬度大調查》，透過網路邀請男性及女性為自己與另一半的硬度表現評分。
根據調查結果，約48%男性自評為小黃瓜、33%自評為帶皮香蕉、14%自評為剝皮香蕉，以及5%自評為蒟蒻。相較於2015年的《兩性健康觀念調查》結果，發現帶皮香蕉男比例減少近2成；而剝皮香蕉男與蒟蒻男比例合計增加約5成，其中蒟蒻男比例還增加超過2倍，可見台灣男性「軟化」嚴重！
勃起硬度分4級 每3位男性就有1位硬度不足
台灣男性學醫學會理事長張宏江指出，勃起硬度不足是常見的男性性功能障礙之一，臨床上勃起硬度分為四級：小黃瓜(四級硬度)、帶皮香蕉(三級硬度)、剝皮香蕉(二級硬度)、蒟蒻(一級硬度)；三級以下即是勃起硬度不足。
根據2010年泛亞太地區調查，台灣約有27%的男性受到勃起功能障礙所苦，依盛行率推估，幾乎每3位男性就有1位有勃起硬度不足的問題。張宏江強調，男性若長期有勃起硬度不足的問題，會影響性愛的頻率，進而影響兩性生活和諧，更會導致自信心低落，影響心理健康。
根據統計，40歲以上男性，約5成有程度不一的勃起功能障礙，其中約7成的發生原因與血液循環不佳有關，與「三高」等慢性疾病有密切關係，導致陰莖血管硬化病變，勃起時血流無法順利進入陰莖微血管而影響勃起。張宏江形容，陰莖像是氣球，一旦血流進不去，等於氣球沒打氣、充不飽，當然硬不起來。
張宏江表示，在勃起功能障礙口服藥物問世前，醫界對於勃起功能障礙治療幾乎是束手無策。直至口服藥物問世後，讓原本是許多男性羞於啟齒的性事，得以透過專業醫師幫助，而不再是隱晦的話題。
導致男性勃起功能障礙的原因來看，可分成三大類，一是器質性，與年齡有關，主要是因為老化影響到神經、血管、荷爾蒙系統，或是外傷、手術、藥物引起；二是心因性，多為心理因素造成，壓力、焦慮、憂鬱等有關；三是混合型，合併心因性與器質性的影響造成。
硬不硬有關係 小心慢性疾病纏身
許多人以為勃起硬度不足只影響性事，張宏江提醒，勃起功能障礙可說是心血管疾病的先期指標。
許多流行病學研究發現，心血管疾病患者中有高比率的勃起功能障礙；也有研究認為，動脈硬化與心血管疾病危險因子皆與勃起功能障礙有關；勃起功能改變，可能是全身動脈粥樣硬化最初的臨床表現；另外患有冠狀動脈心臟病，可能引起勃起功能障礙或使勃起功能障礙加重，不可不慎！建議病患若有勃起功能障礙請盡快就醫，並同時檢測高血壓、高血脂、高血糖等健康指標，了解身體是否有其他異狀。
同時患有三高疾病的勃起功能障礙者，可在醫師的協助之下，選擇勃起功能障礙口服藥物；若用藥後仍未見改善，仍可選擇低能量震波、真空吸引器、陰莖植入手術等方式追求性福。
林口長庚醫院男性學及婦女泌尿科主任陳煜表示，男性若輕忽自身問題，於輕度硬度勃起功能障礙不積極治療，可能會愈來愈軟，因此，提醒有「軟男」前兆，就該積極就醫，以免小弟弟每況愈下「越來越軟」！
垂頭喪氣別擔心 處方用藥好安心
根據2015《兩性健康觀念調查》，4成男性不滿意自己的勃起硬度，其中就有3成曾想上網購買壯陽藥物。陳煜指出，面對勃起功能障礙，很多男性因羞於啟齒而上網尋找方法，反而落入假藥的陷阱，不僅無效且傷荷包又傷身。
調查發現，目前女性對於另一半服藥態度多為正向，認為「對方願意正視、解決自己的性功能問題，很有勇氣」；甚至覺得「對方很用心」；但有超過4成以上女性擔心另一半使用的壯陽藥物傷身或有副作用，提醒男性除勇於面對勃起硬度不足外，更應正確使用藥物。
勃起功能障礙藥物怎麼挑？陳煜提醒，勃起功能障礙口服藥屬處方用藥，建議經醫師診斷，提供專業治療建議，並透過醫療管道取得用藥；藥物必須具有長期追蹤的研究報告，才可知曉其副作用，懂得如何因應；使用最多臨床使用經驗的藥物，除了可提高滿意度，安全性也較有保障。
